Objective

Alameda County Care Connect (AC3) was created to improve outcomes for Medi-Cal beneficiaries with complex needs—individuals often navigating fragmented systems across physical health, mental health, housing, and social services. The program needed a compelling, accessible digital presence to explain its mission, track progress, and connect with stakeholders across sectors.

The Solution

Barretto-Co. partnered with AC3 to design and develop a website that could serve as both an information hub and an advocacy platform.

The goal was to make a complex program understandable and relatable—while supporting transparency and real-time accountability to the community.

We began by clarifying the program’s goals and audiences through stakeholder interviews and content mapping. From there, we created an intuitive site architecture to help diverse users—from policymakers to patients—find what they need quickly.

The design emphasized clarity and accessibility, with a visual system that reflected the program’s commitment to care, equity, and cross-agency collaboration. Key performance indicators and real-time data were integrated to track service expansion and funding allocations.

The final product is a dynamic, user-friendly website that demystifies public health infrastructure and shows how local government can innovate to support vulnerable populations. It became an essential tool for communicating AC3’s values, outcomes, and opportunities to collaborate.
