Alameda County Care Connect (AC3) will develop infrastructure and test programs and incentives to better connect physical health, mental health, substance abuse treatment, and social supports—including housing connections—for vulnerable Medi-Cal beneficiaries with poor health outcomes who are high users of multiple public systems. The AC3 website will be the hub for information about the program and to track its progress.
